Things to Know:

  • Patients are seen by appointment only.
  • Depending on your flexibility, sick visits can usually be seen on the same day or within 24 hours, and well visits within a few weeks.
  • Have your insurance card available when scheduling.
  • Let the scheduler know the nature of your visit.

Bring the Following to Your Appointment:

  • Insurance card
  • Photo ID
  • Co-pay
  • New patient forms can be accessed through our Patient Portal.
  • Medical and or hospital records, if available
  • Bring all prescriptions

If this is your first visit, please bring all medication as well as medical records from your previous primary care provider. If you are a current patient, make sure to keep us posted of any changes to your insurance or billing information. We ask that you arrive 30 minutes prior to your appointment time. Our staff will try to accommodate your needs and make this experience as easy and pleasant as possible.

If you are ill and need to be seen that day, we will make an effort to schedule you to see your primary provider. If they are not available, we will be happy to offer you an appointment to see one of our other highly skilled providers. If you do not cancel or show up for your appointment, there will be a $50.00 fee and this is not covered by your insurance. If you do need to cancel your appointment, please call PR Family Medicine and at least 24 hours in advance.

Please allow 2 business days for prescription refill request to be processed. If you have a prescription filled at the time of your appointment, please allow 2 hours.

Specialist Referrals

Referrals to specialists are generated during business hours and require 48 hours advance notice. Please request a referral at the time of your visit. If you need to request a referral by phone, please contact the office with the specialist’s name and contact information including fax number.

After Hour Emergencies

After Hours Emergencies

We provide 24 hour, 7 day a week coverage for our patients. If you have had a serious injury or believe you may have a life threatening condition, call 911. If you need assistance for an urgent matter after hours, call 301-448-0046 for the on-call practitioner.

Please try to anticipate your provider’s questions and have the answers ready. Be prepared with the details of any medicines you are taking.

Please DO NOT call after office hours for prescription refills, billing problems, or routine appointments.
